- We obtain a CIK (Username) and Password, on your behalf, with the SEC's Edgar Filing System and then complete your Form D filing.

1. A copy of your full PPM 
2. Your Tax ID# 
3. Contact information 
4. Your fiscal year end 
5. A notarized copy of your Articles of Incorporation (Articles of Organization for LLC's).
